Facilitated Resolution
If an Informal Resolution is not available, the student declines the Informal Resolution, or the instructor is unable to get in contact with the student within a reasonable timeframe, the instructor should submit the Academic Integrity Charge Form. In a Facilitated Resolution, a member of the Student Accountability & Conflict Resolution staff will work with instructor and student to try to reach an agreed upon outcome.
End of Semester Notice: In cases where the Academic Misconduct adjudication process is not complete by the time course grades are due, the Faculty Member must assign the student an Incomplete in the course. The course grade will be updated once the case has been resolved.
To refer an alleged violation of academic misconduct to a Facilitated Resolution, please follow the steps below.
Step 1: Contact Student Accountability & Conflict Resolution
- Email accountability@uncc.edu in order to determine what resolution options are available. Please be sure to use your UNC Charlotte email account and include the student’s first and last name, and University ID number.
Step 2: Assign the student an Incomplete
- This will ensure a grade for the course is not issued while there is a pending academic integrity case. If the grading system is not open yet, inform the student that a grade for the course will be issued when the academic integrity process has concluded.
- If a student withdraws after you have informed them of the pending violation, the academic misconduct process may still continue. If this occurs, please contact Student Accountability & Conflict Resolution to discuss this with our staff.
Step 3: Complete the Charge Form
- Complete the Academic Integrity Charge Form.
- The Charge Form can also be found at incidentreport.charlottewp.psapp.dev.
- Select “Report Academic Misconduct.”

- Provide a description of the alleged misconduct. Upload any relevant documentation. If you have documents that cannot be uploaded, please email it to accountability@charlotte.edu.
- Evidence can include, but is not limited to:
- Course syllabus
- Turnitin report
- Assignment(s) or document(s) in question, with the portion containing the alleged violation highlighted
- Witness Information
Step 4: Discuss the case with Student Accountability & Conflict Resolution
- A staff member from Student Accountability & Conflict Resolution will be in contact with you to discuss the case information and potential outcomes.
